"Qdro-Calc: Download the Forms You Will Need"
HERE YOU FIND THE NECESSARY FORMS THAT NEED TO BE FILLED OUT FOR US TO DO THE CALCULATION
NOTE: THERE IS NO CHARGE NOR OBLIGATION FOR YOU TO DOWNLOAD THESE FORMS.
For the QDRO calculation, these downloadable forms will create the table you need to fill in with your account data. It can be done in 2 ways:
(1) download the excel macro file (click TOP RED BOX, it will go to your download folder, save to any different folder you will be able to find, then right click the file and select the properties tab- then check unblock box on the bottom). ALso you may need to allow the file to run. These are precautions windows uses to prevent unwanted macro files from being executed. This file has only the table you will need. Once you fill out the top questions and clicked the create table, the required table should print out as a macro-enabled excel file. Please fill it in and email it back to us using the form on the CONTACT US page.
(2) if you have any issues with getting the table created from the macro, you can just download the questionaire (SECOND RED BOX). Please fill that out and email it back to us using the form on the CONTACT US page and we will create the necessary table for you to fill in and email that back to you.
